- $100 activation fee will apply if the member elects to make monthly payments. This activation fee will be waived if the member elects to make annual payments.

- 4 Perio Maintenance Cleanings
- 2 Fluoride Treatments

Additional Savings with the Membership Plan:
All plan members receive a 15% discount on ALL dental services we provide-NO EXCLUSIONS!
All seniors (65+ years old) receive a 10% discount on basic services. Seniors who join the plan will receive the 15% discount on all services in addition to their senior discount on basic services.
